フリーレイアウトお問い合わせの不具合？ (2021-10-30 23:38 by RAL #88242)

フリーレイアウトお問い合わせを使いたいのですが、フォームの項目を設定しても、プレビューで反映されません。
下記ドキュメントで、「タグをテンプレートに埋め込みます。」とあるので、手順が抜けているのかと思いますが、画像がリンク切れしていて、内容がわかりません。
